I, the undersigned, hereby declare that:

1. I. I have been informed and I give my explicit consent to the non-profit civil company under the name “MAZI SOU MAMA NON-PROFIT CIVIL COMPANY”, hereinafter referred to as the “Company”, for the collection, storage (in electronic or non-electronic form), and processing of my personal data, which will be collected in the context of my participation in the Company’s programs and activities.
2. I consent, agree, and acknowledge as lawful the processing of my personal data, insofar as it is necessary and related to the protection of my professional legitimate interests.

III. I also declare that, prior to and at the time of signing this document, I have been informed of the specific rights granted to me under Regulation (EU) 2016/679 (GDPR), and specifically:

• My right to be informed and to receive confirmation as to whether or not personal data concerning me is being processed, within one month of submitting such a request.
